Athletic Clearance

UHS Marching Band

Due to the physical nature of the activity, all marching band members will need to receive an athletic clearance. This consists of a health screening, physical, and echocardiogram (ECG). Instructions for this process are below.

Summary

  • Each year, you have a physical exam performed by a physician (or Centracare, Nemours, etc.) The physician must complete the Pre-Participation Physical Form.
  • Once for your four years in high school, you must complete an electrocardiogram (ECG.) The physician must complete the FHSAA ECG Screening document.
  • If you have already completed your ECG, you must have the OCPS Cardiology Report filled out.
  • There are four videos everyone must watch on the NFHS Website.
  • You must create an account on the FHSAA Athletic Clearance website and upload these documents before you can participate.

Do Not Wait!

This whole process must be COMPLETED by the first day of Band Camp or you will not be allowed to participate. Get it done now... do not wait until the last minute!

Remember that the ECG is good for four (4) years, but you will need to update the physical form every year.

Please note that you still have to complete the UHS Band Health Packet as well and upload it to your Member Profile right here on the band website (although you can still come to band camp even if this is not done yet.)
