Athletic Clearance

This must be done every year!

Have a licensed physician conduct a physical examination.

This can be done by

• Your primary care provider
• Centracare / Nemours
• Who We Play For (on campus Tuesday, May 2)

The physician who completes your physical exam must complete the FHSAA EL2 Physical Form, linked below:

Note that many providers have their own forms; however, the linked form above is the one that must be completed.

This only needs to be done once and is good for all four years of high school.

Have a licensed physician conduct an electrocardiogram.

This can be done by

• Your primary care provider
• Centracare / Nemours
• Who We Play For (on campus Tuesday, May 2)

The physician who completes your electrocardiogram must complete the OCPS Cardiology Report, linked below:

Note that many providers have their own forms; however, the linked form above is the one that must be completed.

This must be done every year!

• Go to AthleticClearance.com
• Select "Florida."
• Create an account: Parents/Guardians must register with a valid email as the username. Student emails will not work.
• If you have already created an account, log in and select "Start Clearance Here."
• Select the following options:
  • School Year: 2025 - 2026
  • School: University High School
  • Sport(s): Band
• Complete all required fields:
  • Student information
  • Parent/guardian information
  • Medical history
  • Signature forms
  • Required uploads (these are the same forms linked above):
    • FHSAA EL2 Physical Form (all pages)
    • OCPS Cardiology Report
• If you are uploading the OCPS Cardiology Report, check the box that corresponds to your results.
• Sign the bottom and fill in the phone number.
• The cardiac clearance box and cardiologist signature are not required if the ECG is performed at a non-campus screening and the results are normal.
• Once you reach the confirmation message, you have completed the online registration process. This data will be electronically filed with the school's athletic department.
• You will be notified of your clearance status by email. Please note that "Practice Only" is the final stage for marching band and means you are fully cleared!

This whole process must be COMPLETED by the first day of Band Camp or you will not be allowed to participate. Get it done now... do not wait until the last minute!

Remember that the ECG is good for four (4) years, but you will need to update the physical form every year.

Please note that you still have to complete the UHS Band Health Packet as well and upload it to your Member Profile right here on the band website (although you can still come to band camp even if this is not done yet.)
