Online Athletic Clearance
There are two forms you'll need to download and have a doctor fill out: a physical form and an electrocardiogram (ECG) form. Upload these directly to the Athletic Clearance website.
UHS Band Health Packet

Instrument Check-Out Form
This form is for students who are checking out a school instrument to take off-campus at any point.
